Tattoo Request Form

Tattoo Appointments are Wednesdays-Saturdays at 12:00 P.M.

  • Emails are typically replied to in 2-3 days. Please check your spam folder if you have not received a response within one week before resubmitting your request form. Cover-up work is accepted upon review of the tattoo to be covered. Reference pictures can be sent through email once you’ve received a response. Consultations are held in two parts as an initial consult to discuss the details of your tattoo, followed by a secondary consult to review your design and discuss any necessary changes. Consultations are held in-person at the studio, or online via video call. To give each client a unique tattoo, allowing creative freedom in your design concept will achieve the best results. To prevent duplication, designs are not sent out and will be available for viewing during your secondary consultation. If changes are necessary, they will be made before your scheduled tattoo appointment. Current tattoo rates are $175 per hour. A $150 total deposit taken in two parts is required to start any design work and be scheduled in the books. The first half of your deposit is made during your initial consult and the other half during your secondary consult. Deposits can be made online by clicking the “Deposit” button below this request form (Do not place your deposit online unless you have been instructed to do so). Guest Spot / Convention prices are determined based on the tattoo. Deposits will come off the total price on the final session of your tattoo. Deposits are non-refundable and can be forfeited under the following conditions:

    • Canceling or rescheduling on the week of the appointment

    • The skin is freshly damaged at the time of the appointment (cuts, scrapes, burns, etc.)

    • Arriving late to the scheduled appointment time without notice

    Appointments can be rescheduled one time to a later date within 90 days of the previously scheduled date with proper notice. The tattooing process will take several hours, so remember to bring a drink, snacks, charger, headphones, etc. to keep yourself entertained! No minors under 18 years of age.
